Privacy POLICY

Fandor Dental will collect, protect and retain your personal information according to Privacy Policy as well as Confidentiality Standards for our employees.

INSURANCE POLICY

Information required: 

  • Dental coverage details from insurance company and booklet. 

  • Name of insurance carrier. 

  • Policy number, certificate number(ID) and division number (if any). 

  • The anniversary date of the policy e.g. January 1st or rolling calendar year. 

  •  The annual maximum benefit per patient per year. 

  •  The annual fee guide covered by their insurance policy e.g. 2007, 2008, 2009 or current. 

  • Percentage of coverage allowed for diagnostic, preventive, restorative, periodontic services, endodontic and all other major treatment such as crowns, bridges, dentures and number of scaling/ cleaning units covered per year and frequency etc. 

  • The per person and family annual deductible amount.

ELECTRONIC SUBMISSION POLICY

Insurance claim form will be transmitted to your carrier by our computer and get claim acknowledgment form from insurance company indicating the exact amount of the claim for which they will pay and your portion that is not covered. Some claim form submitted manually for that we may need your signatures. As Fandor Dental is an approved assignment office, we request the insurance companies to pay directly. Patients will be requested to pay the balance, if insurance company doesn't pay in full (copayment). The patient will be responsible for full payment to the dental office if the insurance company denies the payment.
